You can use MyChart to view and manage your personal health information, including your COVID-19 records, personal health notes, current health summary, test results, medication information and more.

COVID-19 Records

Go to Your Menu > My Record > COVID-19.

Vaccinations/Boosters

Record your COVID-19 vaccinations/boosters in MyChart and then print or download your vaccination records via a QR code or as a PDF to keep on file or present as proof of vaccination. You may access the same records in this area for any proxy accounts you have access to, such as a child.

COVID-19 Test Results

If you have had COVID-19 tests done at Community, your test results may also be found in this section.

Personal Notes

The Personal Notes feature allows you to add your own secure notes about your healthcare right where you need them most. This is designed for your own record-keeping and is not a way to communicate with your provider(s). Personal notes can only be viewed by you and are not part of your permanent medical record.

You may add personal notes on the following pages:

  • Allergies
  • Medications
  • Problems
  • Immunizations
  • Preventive Care
  • Medical / Social / Surgical / FamilyHistory
  • Family Status

Health Record Summary

To see a summary of your medical record, go to Your Menu > My Record > Health Summary. This summary includes:

  • Current health issues
  • Medications
  • Allergies
  • Immunizations
  • Preventive care

View Test Results

With MyChart, you can view test results as soon as they become available, rather than waiting for a phone call or letter from your physician. You may view test results in two ways:

  1. Go to Test Results in the top menu
  2. Click on Your Menu > My Record > Test Results

Get Notified

To be notified automatically when test results are available in your MyChart account, go to Your Menu > Account Settings > Communication Preferences. Select the Health category to view your current Test Result notification methods. To receive text messages, you may first need to opt in to unlock the text message option. After you've made your selections, make sure to save your changes.

To opt in or out of a communication method: Click on the email (envelope) icon or text (message bubble) icon. If you are opted in, the icon will appear all gray. If you are opted out, the icon will be white with a dark gray outline.

Manage Medications & Refills

To view all your current medications, visit Medications in the top menu or Your Menu > My Record > Medications.

To learn more about a medication, such as side effects, click the "Learn more" link below the medication name.

Request a Refill

  1. To request a refill for one medication, click the "Request refill" link below the medication. To request multiple refills, click the orange "Request refills" button above and to the right ofthe medication list.
  2. Check the box for each medication you want to request a refill for. Then click Next.
  3. Add comments as needed.
  4. Select your pharmacy pick-up location and date/time.
  5. You will receive a message in your MyChart inbox when the prescription refill is processed.

Download Health Record/Lucy Summary

The "Lucy Summary" is a portable version of your health record (including allergies, medications, current health issues, procedures, test results, and immunizations) in MyChart Central.

Link to MyChart Central to download your Lucy record and place it on a USB drive and take it with you to share with your other healthcare providers.

To get started, go to Your Menu > Sharing > Lucy. Check the boxes for information sharing and click "Link me up with Lucy and MyChart Central". You will go to the MyChart Central website to create your account.

Request/Retrieve Medical Records

Request Medical Records

You can use MyChart to request a copy of your medical records. Go to Your Menu > My Record> RecordRequestand fill out the online form.

View RequestedMedical Records

If you've requested a copy of your medical record from your healthcare organization, you can download and view it from MyChart, rather than having to wait for a paper copy to arrive in the mail.

  1. Go toMy Record > Document Centerand choose "Requested Records".
  2. Locate the record you want to view and click "Download".
  3. If the record is password protected, you see a message to warn you. ClickContinue Download.
  4. ClickSaveto save the file to your computer and then open it, or clickOpento open it without saving it to your computer.
  5. If the record is password protected, clickShow Passwordon the Requested Records page to view the password you need to access the document and enter it to view the document.

Flowsheets to Track Daily Health

Your provider might request that you record information such as your daily glucose or blood pressure readings in MyChart. The order is called a"flowsheet" in MyChart.When you enter the information in the flowsheet, your provider automatically receives the readings so he can monitor your progress between visits.

Below are instructions for connecting a flowsheet to a health tracking device such as Google Fit or Apple Health to complete the provider order request.

You may choose how you share your health information with friends and family, other providers and healthcare organizations, and across mobile devices. To review and set your sharing preferences, go to Your Menu > Sharing > Share My Record.

Sharing Options and Preferences

Friends and Family

Control who can see your information, and whose information you can view. This is where you can see which family members' records you can see.

Share Everywhere

Access this feature on the Share My Record page or go to Your Menu > Sharing > Share Everywhere.

Share Everywhere allows you to share your record with just about anyone, like a doctor, chiropractor, social worker, physical therapist, dentist, or school nurse. Grant someone one-time, limited access to the health information you can see in MyChart, including health history, medications, allergies, immunizations, etc. This gives him or her the ability to see your health record summary, view other clinical data, and submit a note to your care team. Link Your Accounts

Access this feature on the Share My Record page or go to Your Menu > Sharing > Link My Accounts.

You can use MyChart to view your health information from other organizations where you and your family have records. When you connect your accounts from the participating organizations, you will be able to see the following types of information in your MyChart account:

  • Allergies
  • Care team
  • Health issues
  • Medications
  • Messages
  • Test results
  • Visits

Connected Apps and Devices

Review which apps you have authorized to view your health data as well as trusted devices that you have linked to your account.

Family Access

If you have access to your family members' medical records, you can view most of the information in their records in the same way that you view your own. Some things that might be useful include viewing a family member's test results or your child's immunization records.

To switch to a proxy record, change the name in the upper right drop-down menu.

Proxy Access to a Family Member's Record

To access a family member's record, you first need permission. After you've received permission, you can view their records by hovering over the icons in the top toolbar and clicking on the appropriate record.

After you read the Proxy Access Disclaimer, click Accept to continue to your family member's chart.

View Your Child's Information

Immunizations/Vaccinations Record

When you are in your child's record in MyChart, go to Your Menu >Health Summary > Immunizations. You can see the immunizations your child has received and the dates on which she received them. Click the immunization name to learn more.

Click the printer icon at the top right to open a print-friendly version.
